The purpose of this procedure is to establish a systematic process for identifying workplace hazards, assessing the associated risks, and implementing effective control measures to prevent injury, illness, and incidents. This procedure is a cornerstone of our proactive approach to safety management.
2. Scope
This procedure applies to all activities, processes, and areas within [Company Name]'s operations, including routine and non-routine tasks. It is applicable to all employees, contractors, and visitors.
3. The HIRA Process
A simple, effective HIRA follows these fundamental steps.
Identify the Hazards: Systematically look for anything in the workplace that has the potential to cause harm. This includes physical, chemical, biological, ergonomic, and psychosocial hazards. Consult with employees doing the work.
Identify Who Might Be Harmed and How: For each hazard, identify the groups of people who are at risk (e.g., operators, maintenance staff, cleaners, public) and how they might be harmed.
Assess the Risk and Prioritize: Evaluate the level of risk by considering the likelihood of the hazard causing harm and the severity of that harm. Use a risk matrix to prioritize hazards that require immediate attention.
Implement Control Measures: Based on the risk assessment, implement control measures using the hierarchy of controls:
Elimination: Remove the hazard completely.
Substitution: Replace the hazard with a safer alternative.
Engineering Controls: Isolate people from the hazard (e.g., machine guards, ventilation).
Administrative Controls: Change the way people work (e.g., safe work procedures, training, signage).
Personal Protective Equipment (PPE): Protect the worker with PPE (e.g., gloves, respirators) as a last resort.
Record, Review, and Update: Record the findings of the HIRA on a formal risk assessment form. Review the assessment regularly (e.g., annually) or when there are changes in the workplace, and update it as necessary.
4. Responsibilities
Management: Responsible for ensuring that HIRAs are conducted and that necessary resources are provided to implement control measures.
Supervisors: Responsible for leading the HIRA process for their areas of control and ensuring employees are aware of the risks and controls.
Employees: Responsible for participating in the HIRA process, following control measures, and reporting any new hazards.
