1. Purpose

The purpose of this procedure is to establish a standardized process for the reporting, investigation, and management of all occupational health and safety incidents, including injuries, illnesses, property damage, environmental spills, and near misses. This ensures timely response, thorough investigation to identify root causes, and implementation of corrective actions to prevent recurrence.

2. Scope

This procedure applies to all employees, contractors, and visitors at all [Company Name] workplaces and project sites.

3. Incident Management Process

  1. Immediate Response: At the scene of an incident, the first priority is to ensure the safety of all persons. This includes providing first aid, making the area safe, and contacting emergency services if required.
  2. Reporting: All incidents, no matter how minor, must be reported immediately to the relevant supervisor or manager. An initial Incident Report Form must be completed within 24 hours.
  3. Investigation: An investigation team, led by a competent person, will be assigned to investigate the incident. The investigation will focus on gathering facts, interviewing witnesses, and identifying the immediate and root causes of the incident. The Incident Investigation Report must be completed.
  4. Corrective and Preventive Actions: Based on the investigation findings, corrective and preventive actions must be developed, assigned to a responsible person, and given a completion date. These actions are tracked to completion.
  5. Reporting to Authorities: For incidents specified in Section 24 of the OHS Act, the Department of Labour must be notified. For injuries on duty, a W.Cl.2 form must be submitted to the Compensation Commissioner.
  6. Review and Close-Out: Once all corrective actions are implemented and verified as effective, the incident investigation will be formally closed out. Lessons learned will be shared across the organization to improve overall safety performance.
